Calvin’s Community Health Worker program is back again!

For several years now, Calvin College’s nursing department has been running the Community Health Worker Program.  There are several Community Health Workers who work for the Creston/Belknap area.  The have been hired from within the community to advocate for and educate their neighbors.  They are paired with senior nursing students and meet individually with neighbors to discuss:
– Health concerns.  Got a medical question?  Try to stump the students!!  We also have a couple FREE HEALTH SCREENINGS if you’re interested.  Might as well get your blood pressure checked while you’ve got the nurses all to yourself.
– Area resources, from prescription assistance to health and dental clinics.  Accessing affordable care is an issue for so many of us, but these teams have a huge compilation of programs and organizations that can help.  They will pair you with the resources that are right for you.
– Issues not directly related to health.  Among the information they’ve gathered, are contact numbers for food pantries, transportation assistance and more.  Health Workers are interested in the big picture.

EVERYTHING IS CONFIDENTIAL!

We will come to your home, or wherever you’re comfortable meeting.  Different health workers are working at different times, so we should be able to fit this into your schedule.  It is a short visit but it can be as in depth as you’d like it to be.  Some folks have a specific concern and that’s all they want to cover.  Others don’t know where to start.  The visit is catered to you, but we know how to get the conversation started.
